Process:

Meet with the client for the initial assessment

Fill in the below form - only 5 sessions can be requested at this point to treat the client​

Wait for approval before continuing to treat the client 

As you progress, you can request a further maximum of 6 additional sessions if required - for these you must email info@therapycentreservices.com with your clinical recommendation and the client reference

The client can complete treatment and discharge or continue self-funded, however at the end of their funded therapy sessions you must complete the discharge form linked at the bottom of the page ​

PLEASE NOTE - The entire course of treatment must not exceed 12 sessions and no further sessions will be approved after this point
